Outpatient PT loses patients twice: once to the missed visit, and again to the plan of care that quietly ends early. The second loss is far larger, and the first one predicts it.
What the research says about PT attendance
A study of outpatient physical therapy attendance in people with chronic pain, published in JOSPT Open, reported a mean no-show rate around 17% and a mean cancellation rate around 10%, with a self-discharge rate of roughly 55%. More than half the cohort left before completing the plan of care.
The same work found that higher no-show rates predicted premature discharge. That is the finding that should change how a clinic runs its schedule: a missed visit is not simply one lost unit of revenue, it is an early warning that the whole episode of care is about to end.
Framed that way, the return on chasing a single missed visit is much larger than the visit fee. You are not recovering one appointment, you are keeping an episode alive that may have eight or twelve visits left in it.
Price the episode, not the visit
Most PT clinics size this problem using the per-visit reimbursement, which understates it badly. The right unit is the remaining plan of care. If a typical episode is twelve visits and the average patient self-discharges after five, every patient who drops out early takes roughly seven visits of authorized, already-referred revenue with them.
Run that against your own numbers. Take your average visits per episode as authorized, subtract your average visits actually delivered, and multiply the gap by your average reimbursement per visit and by your annual patient count.
