Best Patient Reactivation Software in 2026

By , Growth Lead, Puppeteer AI · · 11 min read

Every clinic has a list of patients who were seen once or twice, never booked the next visit, and drifted away. Patient reactivation software works that list for you: it finds the lapsed patients in your records, contacts them, and gets them back on the schedule. The tools differ on the three things that decide whether it pays off: how they find the patients, how they reach them (a text, a recorded call or a real conversation), and whether the visit ends up booked in your EHR or as a task for your front desk.

This guide compares seven of the best patient reactivation software options in 2026, including Recupra, which we make. Every claim about another company comes from that company's own website, checked on October 21, 2026 and linked under Sources at the end. Where a vendor does not publish a price, we say so instead of guessing, and where another tool fits a clinic better than we do, we say that too.

What patient reactivation software does

Reactivation software does three jobs that a front desk rarely has time for:

  • Finds lapsed patients. It reads your EHR or practice management data and builds a list of patients with no recent visit and nothing on the schedule. Some tools use a fixed rule (no visit in 12 months), others look at recall due dates, plans of care or diagnosis codes.
  • Contacts them. By text, email, postcard, a recorded voice message, a live call from your staff, or an AI agent that has the conversation.
  • Books them back. Either it puts the visit straight into an open slot in your EHR, or it sends a booking link, or it hands a list back to your team to call.

Reactivation is not the same as recall. Recall reminds patients who are due for a planned visit, like a six-month cleaning. Reactivation goes after patients who have already lapsed. Several tools on this list do both, and the FAQ at the end covers the difference.

The 7 tools

Recupra (AI voice agent)

Recupra's Reactivation Agent identifies patients past their expected return cadence, prioritized by value and likelihood to return. It then calls them with a check-in personalized to their care history and tone-matched to your clinic, and books the ones who want to come back into a real open slot in your EHR. Every reactivated patient is logged, so you can see what came back.

It connects natively to Healthie, athenahealth, DrChrono, Elation Health, Cliniko, Mindbody, Square Appointments, Acuity Scheduling, SimplyBook.me, Canvas Medical, Medplum and Akute Health, and to 33 more systems, including Epic, Tebra, Jane and SimplePractice, through an assisted setup. The full list is on our integrations page.

Pricing is per result: $40 per dormant patient brought back into care, with no platform fee and no setup fee. In an anonymized beta, 18% of dormant patients were reactivated.

  • Fits: clinics on a supported EHR that want lapsed patients to get a real conversation, not another text, and want to pay only when a patient is back in care.
  • Watch for: we are a young company, and our result is a beta figure rather than years of data. If your EHR is on the assisted list rather than the native one, setup goes through our team.

Brevium (reactivation for specialty medical groups)

Brevium is a dedicated reactivation product, and if you are looking for Brevium alternatives, the rest of this list is a good place to start. It searches your database for patients who have not been seen in a set time, are not on the schedule, ignored recalls or no-showed their last visit, and can also search by ICD-10 and CPT codes. It reaches them by text, email, automated calls, postcards and letters, and its software supports live calls by your staff. Messages can link to online scheduling. Brevium says it connects with "50+ EHR and PM systems", and its specialty pages cover ophthalmology, optometry, dermatology, ENT, cardiology, urology, primary care and others.

Brevium publishes customer results on its own site, such as a dermatology group that "reactivated ~3,000 patients per week across 100+ locations", and says customers see "on average $25 for every $1 spent". It describes its security as "BAA-backed" and HITRUST-aligned and publishes its BAA.

  • Fits: larger specialty medical groups that want broad, multi-channel recall and reactivation across many locations.
  • Watch for: no public price (plans are a fixed annual or variable monthly price), and none of its pages says it books the visit into your EHR for you. Its specialty list does not include dental or physical therapy.

DoctorConnect (automated texts and calls)

DoctorConnect identifies patients who have not had a visit within a set period (6 or 12 months, for example) from your calendar data, with rules that can also use custom fields from your practice management system. It reaches out by SMS, email or automated voice, includes a link or reply option to schedule, and says appointments are booked directly into your existing calendar, with staff notified only when manual help is needed. It says it integrates with "100+ EHR & PM systems" and offers a Business Associate Agreement to every healthcare customer.

  • Fits: US medical, dental, optometry, chiropractic and behavioral health practices on a wide range of EHRs.
  • Watch for: no public price. The results it quotes for reactivation ("20-30% increase in patient return rates compared to staff-only outreach") appear in a blog post without a source.

NexHealth (recall inside a patient messaging platform)

NexHealth's One-Click Recalls read due dates and procedure codes from your health record system to send recall messages automatically, by text and email. Its campaign templates can also target patients who have not been seen in a set time (six months by default). If you buy its Scheduling product, the messages include an online booking link, and NexHealth says it reads and writes to your health record system within 10 to 15 seconds for most integrations. It lists 15 health record systems, mostly dental (Dentrix, Eaglesoft, Open Dental, Denticon) plus athenahealth, eClinicalWorks, ModMed and NextGen Office.

  • Fits: dental and medical practices that want recall, reminders and online booking in one messaging platform.
  • Watch for: its recall pages describe no voice channel, so patients who do not act on texts or emails get no call. Pricing is quote only.

Weave (recall bundled with phones and texting)

Weave has two separate automated message types: a Recall Reminder based on the due date in your practice management system, and a Reactivation message that targets overdue patients by last-seen date. Both skip patients who already have a future appointment. You can send them by text, email or a one-way automated voice message read from a script of up to 625 characters. Online booking from those messages creates requests your staff approve, which Weave then writes back to supported systems. Weave's AI Receptionist can handle replies to recall texts, but its pages do not describe it calling lapsed patients.

Weave publishes a starting price: plans start at $199/month, and recall and reactivation texts and calls are in a $149/month Coordinate Bundle. Weave publishes its BAA.

  • Fits: dental, optometry, veterinary and medical practices that want recall as part of a phone and texting system.
  • Watch for: the voice option is a recorded message, not a conversation, and online bookings wait for staff approval.

Clinekt Health (AI agents in front of your EHR)

Clinekt's Recall Agent scans your records for overdue follow-ups, lapsed care plans and annual exams that never got booked, and defines reactivation as no visit in the prior twelve months and nothing on the schedule. It reaches patients with "a friendly text first, then a call, then an email", in the language each patient prefers. Its physical therapy page describes finding patients who stopped partway through a plan of care. Clinekt says it is HIPAA compliant and SOC 2 Type II, and signs a BAA with every client.

  • Fits: US specialty practices (orthopedics, physical therapy, dermatology and others) that want an AI agent working a recall list across text, phone and email.
  • Watch for: its pages disagree on booking. The home page says it books patients into open slots automatically, while its integrations page says Clinekt "sits in front of your EHR, not inside it" and your team books the appointment as it does today. Its integrations FAQ also says the Recall Agent can work from a list your practice provides. Ask which applies to your EHR. No public price.

Pabau (all-in-one clinic software, UK-founded)

Pabau is a complete practice management system, founded in the UK and sold in the UK, US, Canada, Australia and Europe. Reactivation runs as a marketing automation: the clinic sets rules such as "last appointment 90 days ago, next appointment empty, service is any of these" and Pabau sends SMS or email on a schedule, with a link to book online. Its own guide says it "does not currently offer automated phone call outreach". Every plan includes two workflows; unlimited workflows need the Marketing Plus add-on, which starts at GBP 83 or USD 139 a month on its single-user tier (excluding VAT).

  • Fits: aesthetic, med spa, physio and private clinics, in the UK or elsewhere, that are ready to move their whole practice onto one system.
  • Watch for: it only works if Pabau is your system of record; it does not reactivate patients from another EHR. Its pages show different starting prices for the core subscription, so get a written quote. It says it supports GDPR and offers a data processing agreement; ask where your data is hosted.

Also checked: Cliniko

Cliniko is practice management software for allied health practitioners, and in 2021 it became an official partner of the UK's Chartered Society of Physiotherapy. It has manual recalls that staff work from a report, a report of patients without upcoming appointments, and automated follow-up messages by email or SMS that can go out from one day to two years after an appointment, only to patients with nothing booked. It does not make calls. Plans start at $45 a month for one practitioner, and it says it is UK GDPR compliant and keeps UK accounts' data in the UK. If you are a physio clinic on Cliniko and want lapsed patients called rather than texted, Recupra connects to Cliniko natively.

How to choose

  • Start with your EHR. An add-on tool is only as good as its connection to your system. Look for your EHR named in writing, and ask whether the tool books into it or only reads from it. If you run on Healthie, Akute Health or Canvas Medical, check each vendor's list carefully; Recupra connects natively to all three. WebPT clinics have extra constraints, covered in our guide to patient reactivation for WebPT clinics.
  • Match it to your specialty. Brevium is built around specialty medical groups, NexHealth and Weave lean dental, Clinekt targets specialty and physical therapy practices, and Pabau suits aesthetic, physio and private clinics. A tool that knows your recall intervals and plans of care builds a better list.
  • Decide between text and voice. Texts are cheap, but a lapsed patient may have ignored several already. A recorded call (Brevium, DoctorConnect, Weave) adds a channel. A conversation (Recupra, and Clinekt's call step) can hear why the patient drifted and book them in the same call.
  • Compare pay per result with a subscription. Most tools here charge a monthly or annual subscription whether or not anyone comes back. Recupra charges only per patient reactivated. At $40 per patient, a month where 30 patients come back costs $1,200, and a month where none do costs nothing. Run the same math on your own list before comparing quotes.
  • Get a BAA, or a DPA in the UK. Any vendor that reads your patient list needs a Business Associate Agreement in the US. UK clinics should ask for the vendor's UK GDPR terms and where data is stored. Ask every vendor, us included, before you share a patient list.

If you want to see how many lapsed patients are sitting in your own records, Recupra's patient reactivation page explains how the agent works, and pricing shows what each reactivated patient costs. For patients who missed a visit rather than lapsed, see our comparison of no-show recovery services.

What response rate to expect

There is no independent benchmark for patient reactivation response rates. What is public comes from the vendors themselves, measured in different ways:

  • Weave: across its customers in 2017, "21% of patients contacted with Automated Recall ended up scheduling an appointment." That is recall, not reactivation of long-lapsed patients.
  • Recupra: 18% of dormant patients reactivated in an anonymized beta.
  • Clinekt: "+12%" average lift in appointment volume after go-live, without a method.
  • DoctorConnect: a "20-30% increase in patient return rates compared to staff-only outreach", without a source.

Because each vendor measures something different, none of these numbers can be compared directly. Measure the rate on your own list during a trial, split by how long patients have been gone, and count booked visits rather than replies: a reply that still needs a callback is not a visit yet.

FAQ

Frequently asked questions

What is the difference between patient recall and patient reactivation?

Recall contacts patients who are due for a planned visit, based on a due date in your records, such as a six-month cleaning or an annual exam. Reactivation contacts patients who have already lapsed, usually defined as no visit in a set period (often 12 months) and nothing on the schedule. Weave, for example, runs them as two separate message types. Most clinics need both.

Do I need to switch EHR to use reactivation software?

Usually not. Recupra, Brevium, DoctorConnect, NexHealth, Weave and Clinekt connect to the EHR or practice management system you already use, as long as yours is on their list. Pabau and Cliniko are different: they are practice management systems themselves, so their recall features only work if you run your clinic on them.

Is patient reactivation software worth it for a small clinic?

It depends on how many lapsed patients you have and what a returning patient is worth to you. Pull the number of patients with no visit in the past 12 months and nothing booked. If it is a few dozen, your own staff can call them. If it is hundreds, that is more calls than most front desks can make, and software starts to make sense. Pay-per-result pricing lowers the risk for a small clinic, because a slow month costs little.

Is there patient reactivation software for UK clinics?

Most tools in this list describe HIPAA compliance, the US standard, rather than UK GDPR. Pabau was founded in the UK and offers a GDPR data processing agreement. Cliniko, built for allied health practitioners including physiotherapists, says it is UK GDPR compliant and keeps UK accounts' data in the UK. Ask any vendor for its UK GDPR terms before sharing a patient list.

Why is Recupra on this list?

We make Recupra and we wrote this comparison, so read our entry with that in mind. We kept every other tool's description to what its own website says, linked each source, and noted where a competitor is the better fit.
